Orthognathic surgery repositions the upper jaw, the lower jaw or both so that the jaws relate correctly to each other and the teeth meet properly. It is used when the discrepancy lies in the bone itself, a protruding, receding or deviated jaw, rather than in the inclination of the teeth alone. This is not a single operation that fixes a face in one day. It is a long pathway shared by an orthodontist and a surgeon. Anyone promising a final result from surgery alone, with no orthodontics before and after, is promising something that will not hold. Indications go well beyond appearance: difficulty biting and chewing, uneven tooth wear, an open bite where the front teeth never meet, a visible deviation of the facial midline, difficulty closing the lips, and in selected cases sleep disordered breathing. Improved facial balance is a consequence of correcting the structure, not the only aim. The limits are explicit. Surgery does not move teeth, which is the orthodontist's work. It does not tighten sagging skin or treat signs of ageing, it does not guarantee that existing temporomandibular joint pain will disappear, and it cannot create perfect symmetry, because no human face has two identical halves. Results can partly relapse if retainers are neglected or if a clenching or tongue thrusting habit continues. Planning is built on clinical examination, measurements, lateral and three dimensional imaging, dental models and digital simulation of the planned movement, all reviewed with the orthodontist before any date is set. Candidacy is decided by that assessment, not by lookalike photographs online.
The surgeon and the orthodontist assess together where the problem lies: bone, teeth or both, which jaw will move and in which direction. You discuss what can and cannot be achieved, and whether orthodontics alone would be sufficient.
Lateral and three dimensional radiographs and dental scans are taken, and the planned movement is simulated digitally so distances are calculated and surgical guides produced. This step determines accuracy more than the day of surgery itself.
Braces are placed to align the teeth on each arch independently. The bite may temporarily look worse during this phase, which is expected, and the stage usually lasts 12 to 18 months depending on the case.
The operation is performed under general anaesthesia, usually through incisions inside the mouth so no external scars are visible. The bone is cut precisely, moved to its planned position and fixed with small plates and screws that normally stay in place.
Once the bone has healed, orthodontics fine tunes the final bite over several months, then retainers are fitted and must be worn for years, with regular reviews, since neglected retention is the leading cause of relapse.
Tell your surgeon about all medicines and chronic conditions, any previous anaesthetic problem in you or your family, and any clotting disorder. Complete the tests required before general anaesthesia, and treat decay or gum inflammation beforehand, because a clean mouth lowers infection risk. Stop smoking weeks before and stay off it afterwards, since it delays bone healing. Prepare your home with nutritious liquids, pureed food, a blender, feeding syringes if advised, and pillows to keep your head elevated. Arrange two to three weeks away from work or study and someone to stay with you for the first days. Ask in advance about the pain plan, the diet plan and the date of your first review.
Expect marked facial swelling that peaks on the second and third days then settles over weeks, and numbness of the lip, chin or cheek that may last weeks to months and is usually temporary. Sleep with your head raised, use cold packs as advised, and follow the liquid then gradually softer diet your surgeon prescribes. Clean your mouth gently and thoroughly with the prescribed rinse, because oral hygiene is the single biggest protector against infection. Avoid strenuous exercise and heavy lifting until cleared. **Seek care immediately if you have difficulty breathing or swallowing, bleeding that will not stop, swelling that increases after day three with fever, pus or a foul smell, a sudden change in how your teeth meet, or pain that worsens instead of easing.**
The operation itself usually takes two to four hours depending on whether one jaw or both are moved, with a hospital stay of one to three days. The full pathway, including orthodontics before and after surgery, typically spans 18 to 30 months.
