Ceramic braces are fixed braces whose bonded brackets are made of a porcelain-like material that is tooth-coloured or semi-translucent. They are bonded to the teeth and connected by an archwire running through them, exactly as metal braces are. The essential difference between the two is the bracket material and its appearance, not the mechanics of tooth movement. It matters to separate three things that patients often merge into one: metal braces; ceramic braces, popularly called clear braces, which are fixed and cannot be taken out; and removable clear aligners, which are trays the patient removes and puts back. **Removable clear aligners only move teeth if the daily wear hours prescribed by the orthodontist are respected. An aligner left in its case, or worn for only a few hours a day, means a delayed result or no result at all, however precise the digital plan looked.** Fixed ceramic braces do not depend on that discipline because they are bonded in place and work continuously. The comparison between ceramic and metal deserves a neutral answer. Both can treat most crowding, spacing and bite problems, and neither is universally better for every case. Ceramic brackets are far less visible in photographs and conversation, but they are slightly bulkier, more prone to chipping against hard foods, and the elastic ties around them can pick up stain from coloured drinks between appointments. Metal brackets are smaller, stronger and easier to repair, and they remain the usual choice in complex cases or those needing heavier forces over a longer period. Suitability is decided by clinical examination and by the nature of the case, not by bracket colour. The limits should be stated openly. Ceramic brackets do not shorten treatment time and do not move teeth faster than metal ones; they do not whiten teeth; and they do not make braces truly invisible, since the archwire remains visible. They also change nothing about the most important fact: every form of orthodontic treatment, metal, ceramic or aligner, requires a retainer afterwards to hold the result.
The orthodontist examines the bite and takes radiographs, clinical photographs and arch records to define the malocclusion and judge whether ceramic brackets suit the case or metal ones fit better.
Cavities are restored, the gums are cleaned and any active inflammation is treated first, because bonding brackets in an unhealthy mouth invites decay and mechanical failure.
Brackets are bonded at calculated positions on each tooth, the archwire is secured with elastic ties or clips, and cleaning technique and food restrictions are explained.
The case is reviewed at regular intervals to change wires or ties and monitor movement, and any bracket that has loosened or chipped is replaced.
Once the target bite is reached the brackets are removed and the enamel is polished, then a retainer is fitted the same day to hold the achieved result.
Tell your orthodontist everything relevant about your mouth before treatment begins: cracked or heavily restored teeth, crowns or cosmetic veneers, jaw joint pain, night grinding, or gums that bleed often. Mention any regular medication and any condition that slows tissue healing. Complete fillings and gum cleaning before the bonding appointment rather than after it. If you are considering tooth whitening, ask about timing, because whitening before braces can leave a visible colour difference once the brackets are removed. Prepare practically as well: buy an orthodontic brush, special floss and orthodontic wax before the visit, and plan soft meals for the first couple of days. Most importantly, discuss the ceramic, metal and removable aligner options neutrally, and if you lean towards aligners ask directly how many hours a day you must wear them, because the decision rests on your case and your commitment, not on appearance alone.
Avoid hard and sticky foods throughout treatment: nuts, raw carrot, ice, hard toast and chewy sweets. Cut apples and corn off the cob instead of biting into them, since ceramic brackets chip more readily than metal ones. Brush after every meal with an orthodontic brush and use the floss designed for braces, and cut back on coffee, tea and strongly coloured drinks, which stain the elastic ties. Soreness in the first days after each adjustment is expected and usually settles within a few days; use orthodontic wax over any edge that rubs the lip. Contact your clinic if a bracket comes loose, a wire sticks out or a tie is lost, and never try to cut a wire yourself. **Seek care immediately for increasing throbbing pain, facial swelling, persistent numbness, a white or brown patch appearing around a bracket, or if you swallow a broken fragment.** After treatment, wear your retainer without exception.
The bonding visit usually takes 60 to 90 minutes, while full treatment commonly runs from several months to over two years.
