Treating sensitive teeth means finding the cause of the short, sharp pain triggered by cold, sweet food, air or touch, and then treating that cause rather than masking the symptom. Sensitivity is not a diagnosis in itself; it is a sign that the dentine beneath the enamel has become exposed, so stimuli travel through its microscopic tubules to the nerve inside the tooth. The underlying causes are a familiar list. Acid erosion from fizzy drinks, citrus or reflux thins the enamel. Aggressive brushing with a hard brush carves a notch at the gum line. Gum recession exposes root surface that has no enamel to protect it. Night-time grinding wears the biting surfaces and creates fine cracks. Temporary sensitivity is also normal after scaling, after whitening or after a new filling, and it usually settles over days to a few weeks. One distinction matters more than any other: dentine sensitivity versus an inflamed pulp. Dentine sensitivity is a sharp jolt that disappears within seconds of removing the trigger. If the pain lingers for minutes, arrives on its own, wakes you at night or becomes throbbing, the more likely problem is pulp inflammation, deep decay or a crack, and those need different treatment such as root canal therapy or a restoration rather than a desensitising toothpaste. Treatment starts conservatively. Desensitising toothpastes are used consistently and can be left on the tooth before rinsing. Fluoride varnish or resin sealers are applied in the clinic to block exposed tubules. Damaging habits are corrected, including scrubbing hard and taking acidic food or drink late at night. Where there is a cervical cavity or a leaking filling, the area is restored. Where grinding is present, a night guard is made. With significant recession a gum graft may be offered to cover the root, and root canal treatment remains a last resort when the pulp is damaged. The limits are practical and worth knowing. Desensitising toothpaste needs several weeks to work and does not rebuild lost enamel; it does not treat decay, a crack or an abscess, and relieving sensitivity is no substitute for diagnosing its cause. Salt, lemon and scrubbing with baking soda do not cure sensitivity, they accelerate erosion, and brushing harder makes the problem worse rather than better. Whether any procedure suits you is decided by clinical examination and radiographs, not by reading.

    Characterising and testing the pain

    The dentist asks what triggers the pain, how long it lasts after the trigger is removed, and when it happens, then performs targeted tests: a cold stimulus, a bite test, tapping the tooth, and examination of the gums and roots, with radiographs to rule out decay, cracks and abscesses.

    Identifying the cause, not the symptom

    The cause is classified: acid erosion, a brushing notch at the neck of the tooth, gum recession with an exposed root, grinding, a leaking filling, deep decay, a crack, or transient sensitivity after a recent procedure. Each has a different pathway, and confusing them is the commonest reason treatment fails.

    A desensitising toothpaste is prescribed with a specific way of using it, fluoride varnish or a resin sealer is applied to exposed areas in the clinic, and habits are corrected: a soft brush with light pressure, less acidic intake, and no brushing straight after acidic food or after vomiting.

    Treating the underlying cause

    Cervical cavities and leaking fillings are restored, a night guard is made for grinders, reflux is referred for appropriate treatment, a gum graft may be offered for significant recession exposing the root, and root canal treatment or a crown is carried out when the pulp is damaged or a crack threatens the tooth.

    Follow-up and adjusting the plan

    A review is booked after four to six weeks to measure progress. If the pain has not eased, or its character has changed into constant or throbbing pain, the tooth is reassessed for a deeper cause such as hidden decay, a crack or pulp inflammation, and the plan is changed accordingly.

Before the appointment, note exactly what sets the pain off: cold, heat, sweet food, air, or touching the tooth with a brush, how long it lasts after the trigger is removed, and whether it wakes you at night. Note how many teeth are involved and whether the pain sits in one tooth or across a whole side. Tell the dentist about your habits: brush type and brushing force, fizzy drinks, citrus and lemon juice, smoking, symptoms of reflux or night-time heartburn, grinding or clenching during sleep or under stress, and any recent whitening, scaling or filling. Bring a list of your medications and describe any previous treatment attempt, how long you used it and what happened. Do not use caustic substances, herbal remedies, or scrub your teeth with salt or lemon before the visit, and never place aspirin on a tooth or gum.

After the procedure

Use the desensitising toothpaste exactly as instructed and do not expect a result within days; improvement usually builds gradually over two to six weeks. Brush with a soft brush, light pressure and small circular strokes rather than hard horizontal scrubbing, and clean between your teeth daily. Cut down on fizzy drinks and acidic juices, drink them quickly or through a straw and rinse with water afterwards, and do not brush within half an hour of acidic food or drink or straight after vomiting. If you have a night guard, wear it every night and clean it in cold water. Sensitivity after scaling or whitening is expected and normally settles within days to weeks. Seek care immediately if you develop: **constant or throbbing pain that does not stop when the trigger is removed, pain that wakes you at night, swelling of the gum or face or a taste of pus, sharp pain when biting on a single tooth which may mean a crack, or fever with swelling and difficulty swallowing, which is an emergency**. Also return if sensitivity worsens after weeks of following the plan, or spreads to new teeth.
