A sports medicine visit may address training difficulty or review return to activity after previous care. Describe your sport and affected demands rather than using a teammate’s injury label. For a young athlete, include age, school and family context. This preparation organizes questions; it does not provide an exercise program or clearance to participate without an individual assessment.
Describe your sport and role
Note the sport, participation level, role and situations where difficulty appears. Record recent training changes without assigning a cause.
